Medicaid Waiver
HOME AND COMMUNITY-BASED SERVICES
The Elderly & Disabled Medicaid Waiver program is designed to offer assistance to qualified Medicaid beneficiaries. The program provides services to individuals ages twenty-one (21) or older who, but for the provision of such services, would require placement in a nursing facility. The program allows these individuals to remain in a home or community-based setting. Services include:

Area Agency on Aging (AAA) Services
The Mississippi Department of Human Services, Division of Aging and Adult Services coordinates the delivery of these services available to adults 60 years of age and older. The Area Agency on Aging is leader in lending a helping hand to seniors by providing information and services to enable to live independently. Services include:

CONGREGATE MEALS
Congregate Meals are hot, nutritionally-balanced meals that meet one-third of the current daily recommended dietary allowances for persons over 60 years of age. Meals are served five days a week in senior centers in designated communities.
HOME DELIVERED MEALS
Home-Delivered Meals are nutritionally-balanced meals to the homes of individuals who are unable to leave home without assistance.
Eligibility
Person aged 60 and older must meet certain criteria. An in-home assessment is completed to determine eligibility. First priority is given to individuals who are homebound.

ELDER RIGHTS PROTECTION ACTIVITIES
This program addresses elder abuse, neglect, and exploration. The program includes Outreach, Ombudsman Program, and other assistance for vulnerable adults.

LEGAL ASSISTANCE
Legal Assistance helps senior citizens protect their civil and legal rights by identifying possible legal problems and assisting with legal rights and entitlement by attorneys or legal paraprofessionals.
Eligibility
Any Mississippian 60 years of age or older is eligible for Legal Assistance services. Priority is given to older individuals with greatest economic and social need.

STATE HEALTH INSURANCE PROGRAM (SHIP)
SHIP is a counseling program designed to answer senior citizens’ questions about health insurance, Medicare, Medicaid, supplemental insurance, and other coverage. The SHIP Coordinator can also compare policies, organize paperwork, and help with claims and filing appeals.

NATIONAL FAMILY CAREGIVERS SUPPORT PROGRAM
The National Family Caregivers Support Program is designed to work with family caregivers, grandparents or older individuals who are giving care to relatives. It provides services such as respite care, supplemental services, support groups, training, and information assistance.
Eligibility
Persons eligible are ages 60 and older, family caregivers, grand-parents, or older individuals who give care to relatives. Priority is given to older individuals with greatest social and economic need.

SENIOR COMMUNITY SERVICE EMPLOYMENT PROGRAM (SCSEP)
The Senior Community Service Employment Program prepares older adults, in DeSoto and Tallahatchie counties whose incomes place them at or below the federal poverty level for employment opportunities. Adults in the program generally work an average of 20 hours a week, receiving at least minimum wage.

HOMEMAKER SERVICES
Homemakers assist functionally impaired older persons in their homes by providing housekeeping, laundry, shopping, and other types of assistance with home management tasks.

Information, referral and assistance services provide current facts about resources available in the community and connect individuals with the most appropriate agencies to obtain needed help.

ADULT DAY CARE
Adult Day Care is a place for older persons with serious health problems to impairments to attend during the day where they can experience recreational activities, personal care supervision, nutrition, limited health care, and the opportunity to socialize with others in group or individual activities. It provides care and supervision to individuals who are capable of only limited self-care, and offers care-givers temporary respite from care giving responsibilities.

Persons aged 60 and older who are able to take care of personal needs are eligible. Medicaid Waiver Program clients are also eligible (21 years of age and older).

BRIDGE TO INDEPENDENCE
The Bridge to Independence program assists clients with their transition from a nursing home to their own home or a new residence. Benefits include housing startup costs, home and utility deposits, home modifications, moving expenses and commodities. The plan also works with professionals who can offer therapy services, safety planning, life skills training and caregiver support.
Eligibility
The program can be offered to older adults (65+) and persons with physical, mental, developmental or intellectual disabilities.

OMBUDSMAN PROGRAM
Ombudsman service is the term used to describe the numerous activities in program development and implementation, advocating for, responding to, and investigating and resolving conflicts and concerns made by a long-term care resident or on behalf of the resident.
Eligibility
Any Mississippian 60 years of age or older or a person under 60 years of age who is disabled who resides in or who is a potential resident of a long-term care facility.
